  • What perimenopause is and when it can start
  • Why standard lab ranges may not match your symptoms
  • Looking beyond sex hormones at thyroid, adrenals, gut, and iron

Cassie Milligan is a family nurse practitioner and the owner of Revive Wellness and Aesthetics. She worked as a nurse for a long time before returning to school, choosing family practice because it let her care for patients across the lifespan. Time in primary care shaped how she works now. In an insurance-based model, she explains, a provider has limited minutes and a limited lab panel, and the visit often ends once the basic numbers land inside the reference range.

She opened Revive to bring something to Cache Valley she was not seeing elsewhere: hormone optimization paired with a functional look at the whole body. That means gut health, liver function, adrenals, thyroid, and iron storage alongside sex hormones. She checks ferritin rather than serum iron alone, looks at the full thyroid picture, and times labs to the mid-luteal phase when a patient is cycling regularly. Her practice also includes IV therapy, peptide therapy, and aesthetics, supported by ongoing conference education.

With Bree Allred, Cassie describes perimenopause as the years when hormones decline and fluctuate while the body prepares for menopause, a stretch that can begin as early as 35. She walks through the symptoms women bring in most often, explains what estrogen, progesterone, testosterone, and DHEA do in plain language, and returns to a central idea in this conversation about not feeling like yourself: normal is not the same as optimal.

Ideas worth carrying forward

A few grounded takeaways from Cassie’s conversation with Bree.

Normal lab results and optimal lab results are not the same thing.

Cassie explains that standard reference ranges were largely built from research on white males, and that ranges can differ by ethnicity. A value inside the range may still sit far from where a particular woman feels well.

Symptoms carry information that a single number cannot.

Two people can have the same ferritin value and feel completely different. Cassie pairs a narrower target range with what a patient is actually experiencing, and looks for the level where symptoms ease.

Perimenopause can begin years before menopause and feel unpredictable.

Hormones decline and fluctuate while the body prepares for menopause, which can start as early as 35. That is why one month can bring a heavy period, the next nothing, and the next only light spotting.

Hormone support works better when the rest of the body is considered.

Cassie looks at gut health, liver function, adrenals, thyroid, and stored iron alongside hormones. She checks ferritin rather than serum iron alone, since heavy or erratic bleeding can drain those stores and add to fatigue.

Sleep trouble and persistent fatigue are the symptoms she hears most.

Women often assume the fatigue comes only from poor sleep. Cassie treats both as signals worth investigating further, along with weight gain around the middle, night sweats, brain fog, and new anxiety.
